First Visit
Your initial visit to our office will include a review of your medical and dental histories, a thorough examination and an explanation of our findings. We do not typically perform any therapy at your first appointment, as we want you to have all the information you need and all your questions answered prior to any treatment being provided. Emergency or palliative treatment may be performed in certain cases.
Please assist us by providing the following at the time of your consultation:
- Your referral form and x-rays from your referring dentist.
- A list of medications you are taking.
- Completed dental insurance forms or insurance cards.
- A completed medical history. You may access our online patient registration.
Important: All patients under the age of 18 must be accompanied to their initial appointment by a parent or guardian.
Please alert the office of any significant medical conditions (e.g. diabetes, hypertension, artificial heart valves) or medications (aspirin, anticoagulants, heart medication etc.) before any treatment is received.
X-Rays: Current x-rays are usually necessary to diagnose periodontal conditions and consider implant placement. We will be happy to assist you in obtaining these films from your general dentist. In the event that we need to take x-rays, we utilize digital radiography to dramatically reduce patient exposure.
